产品介绍This gene encodes a polypeptide hormone precursor that undergoes extensive, tissue-specific, post-translational processing via cleavage by subtilisin-like enzymes known as prohormone convertases. There are eight potential cleavage sites within the polypeptide precursor and, depending on tissue type and the available convertases, processing may yield as many as ten biologically active peptides involved in diverse cellular functions. The encoded protein is synthesized mainly in corticotroph cells of the anterior pituitary where four cleavage sites are used; adrenocorticotrophin, essential for normal steroidogenesis and the maintenance of normal adrenal weight, and lipotropin beta are the major end products. In other tissues, including the hypothalamus, placenta, and epithelium, all cleavage sites may be used, giving rise to peptides with roles in pain and energy homeostasis, melanocyte stimulation, and immune modulation. These include several distinct melanotropins, lipotropins, and endorphins that are contained within the adrenocorticotrophin and beta-lipotropin peptides. Mutations in this gene have been associated with early onset obesity, adrenal insufficiency, and red hair pigmentation. ACTH(Adrenocorticotropic Hormone)是由POMC前体蛋白裂解产生的39肽激素,核心功能与研究价值包括:
激素调节枢纽:通过作用于肾上腺皮质束状带,促进糖皮质激素(皮质醇)合成与分泌,参与应激反应、糖代谢及免疫调节;
肿瘤诊断标志物:在垂体ACTH腺瘤(库欣病)、异位ACTH综合征(小细胞肺癌、类癌) 中高表达,IHC检测呈“胞质颗粒状阳性”,是区分垂体源性与异位性库欣综合征的关键依据;
临床检测价值:血清ACTH水平与皮质醇的昼夜节律同步(晨高晚低),其异常升高/降低可辅助诊断肾上腺皮质功能亢进/减退。
实验操作关键要点
IHC-P垂体腺瘤诊断:
抗原修复:柠檬酸盐缓冲液(pH6.0)微波修复10分钟(石蜡切片),垂体ACTH腺瘤细胞胞质呈棕黄色颗粒状阳性(嗜碱性细胞强阳性);
阳性对照:正常垂体组织(前叶促皮质激素细胞散在阳性);
临床意义:与生长激素(GH)、泌乳素(PRL) 抗体联合染色,可精准分型垂体腺瘤(如ACTH⁺/GH⁻提示库欣病)。
ELISA双抗夹心法(血清检测):
包被抗体4℃包被过夜,血清样本1:50稀释(避免基质效应),检测抗体(生物素标记亿欣AH26,0.5μg/mL)37℃孵育1小时;
标准曲线范围0.1-200 pg/mL(临床参考值:晨8时10-52 pg/mL,午夜<10 pg/mL);
干扰排除:避免溶血样本(血红蛋白会干扰HRP显色),高脂样本需12000g离心10分钟。
ICC细胞定位实验:
垂体瘤细胞系(AtT-20)爬片,4%多聚甲醛固定15分钟,0.1% Triton X-100透化10分钟,抗体1:200稀释后4℃孵育过夜,DAPI复染细胞核,可见ACTH在胞质内分泌颗粒中呈“点状聚集信号”。
